Carers requiring help to provide a photograph can contact the WICCF Office: 01859 502588 HS CARER CARD Terms & Conditions About the HS Carer Card HS Carer Card is provided to unpaid carers over the age of 18, who are registered with Western Isles Community Care Forum. It is valid for the period stated on the card, after which it can be renewed by the unpaid carer, if they continue in the role of an unpaid carer. The HS Carer Card remains the property of Western Isles Community Care Forum. All HS Carer Card holders must comply with the following terms and conditions. Terms & Conditions All HS Carer Cards are issued with photographic identification, a start date, an expiry date and a reference number. The HS Carer Card can be used for identification purposes and should only be used by the card holder in conjunction with any discount or service offered by HS Carer Card Scheme Providers, as detailed on our website. Western Isles Community Care Forum reserve the right to change the appearance and conditions of the HS Carer Card or withdraw the HS Carer Card Scheme at any time. Carers who no longer provide unpaid care due to the person they cared for going into residential care, should notify Western Isles Community Care Forum as soon as possible and return their card. Carers who are no longer caring due to a bereavement, can use their card for 12 months following the bereavement. If you experience any difficulty in using your HS Carer Card, then please let us know immediately so that we can try to resolve the issue. If you have lost or misplaced your HS Carer Card, please contact Western Isles Community Care Forum and a replacement card can be issued. There may be a charge for issuing replacement cards. Misuse of Card Western Isles Community Care Forum and HS Carer Card Scheme Providers have the right to refuse or withhold any HS Carer Card if they suspect it is being used inappropriately or fraudulently. HS Carer Card Scheme Providers reserve the right not to provide goods and services, offered under the terms and conditions of this agreement, if no card is presented or if a card is invalid. HS Carer Card Scheme Providers HS Carer Card Scheme Providers agree to provide the goods and services detailed within the agreement made with Western Isles Community Care Forum, to any person(s) in receipt of a valid HS Carer Card issued by Western Isles Community Care Forum. In providing these goods and services, HS Carer Card Scheme Providers agree to do so in a way that does not constitute inappropriate, offensive, discriminatory, illegal, fraudulent, harmful or unlawful behaviour or practice. Western Isles Community Care Forum reserves the right to report any such wrongdoing to relevant governing bodies and/or authorities. Western Isles Community Care Forum cannot recommend or endorse the products, services or commercial offerings of any third party. By giving you information on our website about the providers or services, we are merely providing you with an opportunity to find out more about the special offers available to carers which may be of interest. These offers may change from time to time without advanced warning. If they do capture any personal information about you, they must do so with your explicit consent. Please notify Western Isles Community Care Forum at your earliest convenience if your personal details or caring circumstances change. Changes to these terms and conditions We reserve the right to change the terms and conditions of this agreement at any time. If we do so we will notify you in writing at least 30 days before making any such changes, providing details of the changes made.
